Office declaration for a declined HDFC Pre-Approved Card (Moneyback+) Existing Classic customer with High amount FD

I am existing Classic customer with high amount FD

It was a pre-Approved Card with 75000 Limit only and at the time of application no office address was asked

KYC fetched through Internet Login

I need Home delivery of Card (not at office)

on 22.07.2024 got an email from grievance.redressalcc @ hdfcbank.com

in the email they are asking for office address declaration and sent a pdf file as follows:
