you can also apply through HDFC branch but branch executive should apply the Code: RRR01 in ATM/Debit card form and also told them wrote a mail to head office Specifying the card name (i.e RUPAY PREMIUM DEBIT CARD).

Question 1) This is how the form is getting printed. Will this work? It says form will be read by machine/scanner. But in my printout there is no surrounding box around FULL NAME. So then how will the machine read it?
Question 2) In the form you provided in main post, there is no option of EasyShop Platinum on page 2. There is only Easyshop Regular or Easyshop Gold. SO then what to choose?
Question 3) Do we have to fill only Name, Acc Number, Cust ID, RRFL1 code? Or anything else also?

Update: They accepted the form but the front desk lady said they would charge me ₹500 or so.🙄I pointed out to her that the RuPay card with RRFL1 was free and besides I was a SavingsMax account holder as well as a classic banking customer. She tried her best to keep me on Visa but relented. Hopefully they won’t actually charge or I’ll have to file a grievance to get it back.

@abhishek012 Bro is RRFL1 free Rupay Platinum DC available for Regular 5k MAB semi-urban savings account ? If they deny which code is for the paid one and what are the charges ? I couldn't find the answer anywhere.

I was waiting to pickup a friend today. Saw that HDFC bank branch was just where I was waiting. I walked in and the guy was super helpful. It wasn’t my home branch. The guy at first told me Rupay Platinum is not available, it has been stopped. So I told him the code and he said it is Rupay Premium, not Platinum. So I told him to issue it as I knew its the same card. The card will be delivered in a week.
One more RuPay Platinum available on HDFC bank. I dnt knw which card it is, only thing i know is card BIN number.
I believe HDFC is the one and only bank where the Visa Millennia debit card is more rewarding that RuPay Platinum/RuPay Premium. For all other banks, Visa cards can't beat RuPay Platinum due to Amazon and Swiggy offers. I wish Millennia launched on RuPay network.
I opened account in hdfc bank and told customer care people to issue rupay platinum
To my suprise i got millinea card with 590₹ charge deducted from my account
My question is if i apply RRR01 code and apply rupay premium card at branch then will they reverse back millinea debit card fees and deduct 200+gst for rupay card or will they charge again one more fees of 200+gst ..Please reply thanks
I already have SavingsMax with Millennia DC. If I wish to open another account with minimum/low MAB online just for this RuPay card, and wish to avoid dealing with HDFC bank branch staff, on what type of account will this RuPay Premium be issued? Can I open that account online? Because I doubt branch staff will open PMJDY account for SavingsMax account holder with Millennia and Classic banking and 2 HDFC CCs 
How did you get Millennia dc with SavingsMax Account?
Isn't Millennia DC given only to HNI or preferred accounts with 1L AMB only?
@abhishek012 as per your guidance I personally went to HDFC BANK branch at Rajouri Garden, Delhi. I filled blue pre-printed form on 03/4/2023. Applied RRFL1 code. Write RUPAY PLATINUM DEBIT CARD. Afraid of not written premium card. But got today i.e. 11/4/2023 same card as shown in topic. But customer care official told me it will be charged ₹200+gst. Lot of thanks for proper guidance and thanks all other desidimers bro. you have made topic so easy anyone can understand easily to read step by step.

I have requested millennia on my savings max acc but they are giving easyshop platinum (able to see virtual card in app)tried 2-3 times ,generated easyshop platinum only,how do i get millennia?
The form link is not working. Can anyone please share working link. Also, please confirm RRFL1 code is still valid for free card.
