Need a laptop repair bill, preferably for a macbook

Could someone please share a repair invoice in PDF format for a MacBook or any other laptop repair? You can redact any personal information—I just need a template to submit to my office.

Reason: My MacBook's battery is depleted and it overheats frequently, which has started to impact my daily work. I raised a request with my company, and they agreed to reimburse up to ₹5,000 for repairs, as it's my personal device (we're a startup and have to use our own equipment). Instead of getting it repaired immediately, I'm using it while plugged in and plan to do the repairs later when the battery is completely unusable. Hence, I require an invoice.
